This form is exclusively for those buying using either a Limited Company or LLP and must be completed even if you have also completed the Partnership Forms if the NHS Contract you are acquiring is outside the Limited Company but you intend to process private treatments through the Limited Company. If the Nominated Individual or Registered Manager Applicant does not already have or have not already applied for a CQC-Countersigned DBS Check that is less than ten months old, please urgently click the Get DBS Check button above and apply for the check as it may take up to four weeks and will hold up the CQC Application process.
