This form is exclusively for those either buying a practice not through a Limited Company and/or those who are buying a practice with an NHS Contract where the NHS contract is not incorporated. If either of these situations do-not apply please contact our Sales Progression team before completing the forms below.
If you or any of your partners do not already have or have not already applied for a CQC-Countersigned DBS Check that is less than ten months old, please urgently click the Get DBS Check button above and apply for the check as it may take up to four weeks and will hold up the CQC Application process.
