This form is vendors and partners who are in the process of selling their dental practice. Please do-not complete this form if you are selling your Limited Company to the prospective purchasers and any NHS contract you hold is held within this Limited Company. If the NHS Contract is outside of the Limited Company you will need to complete this form. If you have any queries please contact your practice consultant.
